Adenovirus infection, conjunctivitis, hemorrhagic cystitis, and pharyngitis.

 

Description of Adenovirus Immunofluorescence

A group of virus types responsible for upper respiratory tract disease, hemorrhagic cystitis, and epidemic keratoconjunctivitis. The mode of transmission is by direct or indirect contact. Direct immunofluorescence is performed for adenovirus group antigen on a swab or scraping.

Preparation

  1. Obtain a sterile microbiologic swab or scraper and viral transport media. For a tracheal specimen, obtain a sputum trap and a suction cannula.

 

Procedure

  1. Upper respiratory tract disease, nasopharyngeal specimen: Advance a curved swab through the nares to the posterior pharyngeal wall and leave it in place for 10 seconds. Remove the swab.
  2. Pharyngeal specimen: In a well-lit location with the client's head tilted back and the mouth open, depress the tongue with a tongue blade. Have the client say “Ah.” Swab the posterior pharyngeal wall with a curved wire swab from one side to the other. Swab any lesions or drainage noted. Remove the swab without touching it to any other portions of the oropharynx or mouth.
  3. Tracheal specimen: Obtain a 1- to 3-mL sample of tracheal aspirate by oral or nasotracheal suction directly into a sterile sputum trap.
  4. Hemorrhagic cystitis: Swab or scrape urethral discharge (or a vesicle lesion visualized by cystoscopy). Alternatively, using a small platinum loop, scrape the inside of the anterior urethra.
  5. Keratoconjunctivitis: Do not rinse the eye or apply medication to the conjunctiva before specimen collection. Swab or scrape the inflamed surface of the inner, lower conjunctiva or the inner canthus of the eye.

 

Postprocedure Care

  1. Place the swab or scraping in viral transport media or a tightly capped tube containing 2 mL of 1% bovine serum albumin or veal infusion broth.
  2. Refrigerate the specimen.

 

Client and Family Teaching

  1. Sampling should be painless.

 

Factors That Affect Results

  1. Failure to send the specimen to the laboratory immediately or to immediately chill the sample.

 

Other Data

  1. This test is nonspecific for the type of adenovirus present.
