Contraindications
Sensitivity to homatropine or components, primary glaucoma or narrow angle glaucoma
Cautions
Discontinue the drug if signs or symptoms of sensitivity occur during treatment or if irritation persists or increases
Use with extreme caution in infants and young children because of increased susceptibility to systemic effects
Excessive use may cause CNS adverse effects including delirium, confusion, coma, or agitation
Use appropriate eye protection (may cause sensitivity to light)
Use caution in Down syndrome patients (susceptible to angle closure glaucoma)
Use caution in patients with keratoconus; may result in fixed pupil dilation
Elderly are susceptible to side effects; use caution
Use caution in infants and young children due to susceptibility of systemic side effects
Discontinue if experiences pain within the eye
Pregnancy and lactation
Pregnancy category: C
Lactation: Excretion in milk unknown; not recommended
Pregnancy categories
A: Generally acceptable. Controlled studies in pregnant women show no evidence of fetal risk.
B: May be acceptable. Either animal studies show no risk but human studies not available or animal studies showed minor risks and human studies done and showed no risk.
C: Use with caution if benefits outweigh risks. Animal studies show risk and human studies not available or neither animal nor human studies done.
D: Use in LIFE-THREATENING emergencies when no safer drug available. Positive evidence of human fetal risk.
X: Do not use in pregnancy. Risks involved outweigh potential benefits. Safer alternatives exist.
NA: Information not available.
Pharmacology of Homatropine (Isopto Homatropine), homatropine ophthalmic (homatropine)
Mechanism of action
Blocks the responses of the sphincter muscle of iris & ciliary muscle of the lens to cholinergic stimulation, thereby producing mydriasis and cycloplegia
