Usage of 11-Deoxycortisol (Compound S, 11-DOC) Test

Basal levels aid in diagnosis of adrenal carcinoma and congenital adrenal hyperplasia; used to measure response when metyrapone is given to diagnose adrenal insufficiency and Cushing's disease. Low levels found in amyloidosis and posttraumatic stress disorder (PTSD). High levels found in adrenocortical tumors.

 

Description of 11-Deoxycortisol (Compound S, 11-DOC) Test

Blood test used to determine a specific metabolic block in the synthesis of cortisol. Cortisol is synthesized by two successive hydroxylations of 17-alpha-hydroxyprogesterone. The first results in 11-deoxycortisol, which is then catalyzed by 11-beta-hydroxylase to yield cortisol. This test is used with the metyrapone test to allow diagnosis of primary and secondary adrenal insufficiency. Metyrapone blocks the conversion of 11-deoxycortisol to cortisol, which then stimulates the adrenals to produce more 11-deoxycortisol. A blood level of 11-deoxycortisol that is not elevated after metyrapone administration indicates the presence of adrenal insufficiency.

  1. Tube: Green topped.

 

Procedure

  1. Obtain a 7-mL blood sample.

 

Postprocedure Care

  1. Deliver the specimen to the laboratory immediately. Separate and freeze the plasma.

  1. Results are normally available within 48 hours.

 

Factors That Affect Results

  1. Results are increased if the client is taking any glucocorticoids, such as hydrocortisone, dexamethasone, or prednisone.

 

Other Data

  1. 11-Deoxycortisol has no glucocorticoid activity.
