Talwin Nx (pentazocine hydrochloride and naloxone hydrochloride) is a combination of a narcotic pain reliever (opiate-type) and an opioid antagonist used to treat moderate to severe pain.

What Drugs, Substances, or Supplements Interact with Talwin?

Talwin Nx may adversely interact with:

  • naltrexone,
  • sibutramine,
  • fluoxetine,
  • MAO inhibitors,
  • alcohol,
  • anti-seizure drugs,
  • medicine for sleep or anxiety,
  • muscle relaxants,
  • psychiatric medicines or
  • any medication that may cause drowsiness or affect breathing

Talwin During Pregnancy and Breastfeeding

During pregnancy, this medication should be used only when prescribed. Using it near the expected delivery date is not recommended because of possible harm to the fetus. Discuss the risks and benefits with your doctor. Infants born to mothers who have used this medication may have withdrawal symptoms such as irritability, abnormal/persistent crying, vomiting, or diarrhea. Tell your doctor immediately if you notice any of these symptoms in your newborn. It is unknown if this product passes into breast milk. Discuss the risks and benefits with your doctor before breastfeeding.

Additional Information

Do not stop using Talwin Nx suddenly after long-term use, or you could have withdrawal symptoms.

Talwin Nx Consumer Information

Get emergency medical help if you have signs of an allergic reaction (hives, difficult breathing, swelling in your face or throat) or a severe skin reaction (fever, sore throat, burning in your eyes, skin pain, red or purple skin rash that spreads and causes blistering and peeling).

Opioid medicine can slow or stop your breathing, and death may occur. A person caring for you should give naloxone and/or seek emergency medical attention if you have slow breathing with long pauses, blue colored lips, or if you are hard to wake up.

Call your doctor at once if you have:

  • noisy breathing, sighing, shallow breathing, breathing that stops;
  • a slow heart rate or weak pulse;
  • a light-headed feeling, like you might pass out;
  • severe constipation;
  • confusion, hallucinations, unusual thoughts or behavior;
  • a seizure;
  • low cortisol levels--nausea, vomiting, loss of appetite, dizziness, worsening tiredness or weakness; or
  • serotonin syndrome--agitation, hallucinations, fever, fast heart rate, muscle stiffness, twitching, loss of coordination, nausea, diarrhea.

Serious breathing problems may be more likely in older adults and those who are debilitated or have wasting syndrome or chronic breathing disorders.

Common side effects may include:

  • dizziness, drowsiness;
  • constipation;
  • stomach pain, nausea, vomiting; or
  • feeling tired.

Talwin Nx Professional Information

SIDE EFFECTS

Cardiovascular

Hypertension, hypotension, circulatory depression, tachycardia, syncope.

Respiratory

Rarely, respiratory depression.

Acute CNS Manifestations

Hallucinations (usually visual), disorientation, and confusion .

Other CNS Effects

Grand mal convulsions, increase in intracranial pressure, dizziness, lightheadedness, hallucinations, sedation, euphoria, headache, confusion, disorientation; infrequently weakness, disturbed dreams, insomnia, syncope, and depression; and rarely tremor, irritability, excitement, tinnitus.

Autonomic

Sweating; infrequently flushing; and rarely chills.

Gastrointestinal

Nausea, vomiting, constipation, diarrhea, anorexia, dry mouth, biliary tract spasm, and rarely abdominal distress.

Allergic

Edema of the face; anaphylactic shock; dermatitis, including pruritus; flushed skin, including plethora; infrequently rash, and rarely urticaria.

Ophthalmic

Visual blurring and focusing difficulty, miosis.

Hematologic

Depression of white blood cells (especially granulocytes), with rare cases of agranulocytosis, which is usually reversible, moderate transient eosinophilia.

Dependence and Withdrawal Symptoms

Other

Urinary retention, paresthesia, serious skin reactions, including erythema multiforme, Stevens-Johnson syndrome toxic epidermal necrolysis, and alterations in rate or strength of uterine contractions during labor.
